Breast Reduction: A Lighter Body, A Freer Silhouette

Vox Aesthetic: Doctor, in this summer issue we are discussing breast reduction. In one sentence, what does this operation mean to you?

Dr. Serdar Bora Bayraktaroğlu: Breast reduction is not simply an operation that makes the breasts smaller; for many patients it is a powerful transformation that lightens their relationship with their body, improves posture and makes daily life more comfortable. Large breasts may look like an aesthetic concern from the outside, but they can also cause neck, back and shoulder pain, deep bra strap marks, irritation beneath the breast fold, difficulty choosing clothes and posture problems. That is why breast reduction is one of the special procedures in aesthetic surgery where beauty and comfort meet.

Vox Aesthetic: Why can large breasts become a problem over time?

Dr. Serdar Bora Bayraktaroğlu: In some women, the breasts are disproportionately large from puberty onward. In others, pregnancy and breastfeeding increase breast volume, and the expected reduction afterward may not occur. Aging, weight fluctuations and loss of skin elasticity may also lead to both enlargement and sagging. The issue is not only “large size.” As the breast tissue becomes heavier, gravity pulls it downward, the upper body appears broader, and the person may look older or heavier than she actually is. This can affect daily life both physically and emotionally.

Vox Aesthetic: What are the most common complaints patients bring to you?

Dr. Serdar Bora Bayraktaroğlu: The pattern is very clear: back pain, neck pain, deep bra strap grooves on the shoulders, rash and redness under the breasts, sweating in summer, difficulty exercising, trouble finding clothes and the constant need to camouflage the body. Some patients say, “I have not been able to stand upright for years.” Others have worn oversized clothes since youth to hide their chest. This is where the most impressive part of breast reduction begins: the patient does not only want smaller breasts; she wants a more comfortable body, a freer posture and a lighter everyday life.

Vox Aesthetic: So can we say breast reduction is not only an aesthetic procedure?

Dr. Serdar Bora Bayraktaroğlu: Absolutely. Breast reduction is an aesthetic operation, but it is not only aesthetic. We perform this surgery to improve body proportion, create a younger and fitter chest contour, reduce the physical load on the upper body and increase quality of life. Of course, we aim for a beautiful, firm breast shape that suits the body; but we also care that the patient can walk more comfortably, exercise more easily, wear summer clothes more freely and feel better when she looks in the mirror. In that sense, breast reduction may also be described as a “lightening operation.”

Vox Aesthetic: Is there such a thing as the ideal breast size?

Dr. Serdar Bora Bayraktaroğlu: I do not believe the ideal breast size can be described with a fixed measurement. The ideal changes according to the patient’s height, weight, chest width, shoulder structure, waist-to-hip ratio, skin quality and expectations. A size that looks natural and elegant on one person may look too small or too large on another. For me, the key word is proportion. In breast reduction, the goal is to design a breast shape that suits the patient’s body, makes her look lighter and more balanced, and remains wearable for years.

Vox Aesthetic: How do you plan the surgery before the operation?

Dr. Serdar Bora Bayraktaroğlu: First, we perform a detailed examination. We evaluate breast size, degree of sagging, nipple position, skin quality, chest wall structure and any asymmetry between the breasts. Then we listen carefully to the patient’s expectations: Does she want a more athletic look, a more natural form, or a silhouette that feels easier in clothing? In breast reduction surgery, an aesthetic eye is as important as measurement. We are not only removing tissue; we are rebuilding the architecture of the breast.

Vox Aesthetic: In the original scenario, you mention a busy patient in her mid-forties. How do you approach such a case?

Dr. Serdar Bora Bayraktaroğlu: In patients like this, two needs often overlap: physical relief in daily life and a younger, more balanced upper-body appearance. The patient I described had spent years camouflaging her chest with loose clothing and had gradually developed a forward-leaning posture. When breast enlargement was combined with sagging, the aesthetic appearance was significantly affected. During examination, we designed a new breast form that matched her height, weight, rib cage and lifestyle. After surgery, not only did her breasts become smaller; her posture, the way she wore clothes and the way she saw herself also changed.

Vox Aesthetic: Technically, what happens during breast reduction surgery?

Dr. Serdar Bora Bayraktaroğlu: During the operation, excess breast tissue, fat and skin are removed. The nipple is moved to a higher and more aesthetic position. The remaining breast tissue is reshaped to create a smaller, firmer and more lifted form. In other words, this is not only a “reduction” procedure; it is also a reshaping operation. The nipple, breast cone, lower breast fold and overall silhouette are considered together. A successful result is not a small but lifeless breast; it is a natural, lively breast shape that suits the body.

Vox Aesthetic: Is the nipple completely removed and placed somewhere else?

Dr. Serdar Bora Bayraktaroğlu: In most standard breast reduction techniques, the nipple is moved upward while preserving the blood supply, nerve connections and tissue bridge that keep it alive. The goal is to reposition the nipple while maintaining its vitality, sensation and function as much as possible. However, every patient has a different breast size and degree of sagging, so the surgical technique must be individualized. In very advanced cases, different techniques may be necessary. This is why preoperative examination and precise surgical planning are so important.

Vox Aesthetic: Scars are one of the subjects patients ask about most. Is breast reduction scarless?

Dr. Serdar Bora Bayraktaroğlu: No, breast reduction is not a scarless operation. It is important to be honest with the patient from the beginning. However, where the scars are placed, how they are planned and how they mature over time are closely related to the surgical technique and the healing process. Scars are generally planned around the areola, vertically from the areola downward, and when necessary, within the fold under the breast. The lower breast fold is already a natural transition area. Over time, scars usually fade, soften and become less noticeable. Most patients find them acceptable when they experience the new shape and comfort they have gained.

Vox Aesthetic: How do you answer the question: “If there will be scars, is it worth it?”

Dr. Serdar Bora Bayraktaroğlu: This is a very human question. I tell my patients that scars are a reality in breast reduction, but so are the weight, sagging, pain, irritation, posture problems and loss of confidence caused by overly large breasts. When patients move more comfortably after surgery, wear summer clothes with greater ease and realize their bra straps no longer cut into their shoulders, many say, “I wish I had done this earlier.” The decision is about understanding the balance between scars and quality of life.

Vox Aesthetic: What is the postoperative period like?

Dr. Serdar Bora Bayraktaroğlu: The procedure is performed under general anesthesia, and most patients stay in the hospital for one night. At the end of the operation, a special medical bra is worn. In the first days, mild to moderate swelling, tightness, bruising and tenderness are expected. If necessary, I may use small drains; these are usually removed after a short time. During the early period, the patient rests on her back with the upper body slightly elevated. Around one week, we perform a control visit and evaluate the tapes. Return to social life is often quick, but heavy exercise, lifting and movements that strain the chest area should be resumed gradually and carefully.

Vox Aesthetic: Is the surgery painful?

Dr. Serdar Bora Bayraktaroğlu: Pain after breast reduction is usually more manageable than patients expect. Most patients describe tightness, pressure, tenderness and a pulling sensation with movement rather than severe pain. Regular use of prescribed medication, wearing the medical bra correctly and avoiding strain in the early days make the recovery much more comfortable. Of course, every patient’s pain threshold and healing pace are different, so the process is followed individually.

Vox Aesthetic: When can patients return to normal life?

Dr. Serdar Bora Bayraktaroğlu: Patients with desk jobs or a light daily routine can usually return to social life in a controlled way after about one week. However, this does not mean that everything is immediately allowed. In the first weeks, patients should avoid heavy lifting, sudden arm movements, sleeping face down and should wear the recommended medical bra regularly. Sports, swimming, heavy exercise and upper-body workouts should usually wait until later weeks. In breast reduction surgery, returning quickly is important, but healing well is even more important.

Vox Aesthetic: Is the result visible immediately?

Dr. Serdar Bora Bayraktaroğlu: The new form is visible immediately after surgery; the patient sees a smaller and higher breast shape right away. But this first appearance is not the final result. Swelling decreases, tissues soften and the breasts gradually settle. In the first weeks, the breasts may feel firmer and higher; over the following months, they achieve a more natural shape. Scar maturation also takes time. Therefore, in breast reduction, the result begins in one day but becomes more refined over months.

Vox Aesthetic: Can breast reduction affect breastfeeding?

Dr. Serdar Bora Bayraktaroğlu: This subject should be discussed very clearly and honestly. Breast reduction may not affect breastfeeding in some patients, while in others it may reduce milk-producing capacity. This depends on the surgical technique, the amount of tissue removed, the way the nipple is moved and the patient’s own breast tissue characteristics. For patients who are planning pregnancy and breastfeeding, I discuss this issue in detail before surgery. The aim is to create the safest plan both aesthetically and functionally.

Vox Aesthetic: Does breast reduction affect breast cancer screening?

Dr. Serdar Bora Bayraktaroğlu: Patients who undergo breast reduction should continue their routine breast checkups. Depending on age and risk factors, ultrasound, mammography or breast surgery evaluation may be recommended. Before surgery, especially in patients over a certain age, we also want to see appropriate breast imaging. After any surgery on breast tissue, radiological evaluations may show surgical scars and tissue changes; therefore, patients should always inform their physicians about their surgical history.

Vox Aesthetic: Who is a good candidate for breast reduction?

Dr. Serdar Bora Bayraktaroğlu: Patients may be good candidates if their breasts are large for their body frame, if they experience back, neck or shoulder pain, irritation beneath the breast fold, difficulty exercising or choosing clothes, breast sagging, and a desire for a lighter upper body. Realistic expectations are also essential. Breast reduction can transform the body, but it is not a magic wand. When the surgeon and patient meet at the same goal, the result is much more satisfying.

Vox Aesthetic: What is the most important thing to explain to the patient before surgery?

Dr. Serdar Bora Bayraktaroğlu: Three things must be explained clearly. First, this operation is planned individually; there is no single size, technique or scar pattern for everyone. Second, there will be scars, but with correct planning and good care they usually become more acceptable over time. Third, the result can be enjoyed for many years, but weight changes, pregnancy, aging and skin quality may affect the breast shape over time. Realistic expectation is the key to satisfaction in breast reduction.

Vox Aesthetic: Are the results long-lasting?

Dr. Serdar Bora Bayraktaroğlu: The result of breast reduction can be maintained for many years. Since excess tissue is removed and the breast is reshaped, the patient can enjoy a lighter, smaller and more lifted breast form for a long time. However, the body continues to live and change. Aging, gravity, weight fluctuations, hormonal periods and pregnancy can affect breast tissue. Maintaining a stable weight, exercising, using good bra support and taking care of the skin help preserve the result longer.

Vox Aesthetic: Can this operation be combined with other procedures?

Dr. Serdar Bora Bayraktaroğlu: Yes, in suitable patients, breast reduction can be part of a broader body-contouring plan. For example, it may be combined with tummy tuck, liposuction or other contouring procedures. However, the decision to combine surgeries depends on the patient’s general health, operation time, blood values, healing capacity and safe surgical limits. In aesthetic surgery, the goal is not to do more procedures, but to combine the right procedures safely and harmoniously.

Vox Aesthetic: Does breast reduction have a special meaning for the summer season?

Dr. Serdar Bora Bayraktaroğlu: Summer is one of the seasons when the chest area becomes most visible. Thin fabrics, swimsuits, strappy dresses and light-colored clothes reveal breast shape more clearly. Patients with large and sagging breasts often feel the need to hide themselves even in summer. After breast reduction, patients choose swimsuits more comfortably, wear lighter clothes more easily and complain less about sweating and irritation under the breasts. So breast reduction changes not only the mirror image, but also the relationship a person has with her body in summer.

Vox Aesthetic: What changes psychologically after breast reduction?

Dr. Serdar Bora Bayraktaroğlu: Many patients say after surgery, “I feel like my body belongs to me.” This is a very valuable sentence. Large breasts can sometimes attract unwanted attention, make a person hide her body or cause her to be perceived differently than she wishes. After surgery, the patient stands more upright, feels freer when choosing clothes and moves more comfortably during exercise. Sometimes, what becomes smaller is not only the breast; the invisible burden the patient has carried for years also becomes lighter.

Vox Aesthetic: Finally, how would you summarize breast reduction in one sentence?

Dr. Serdar Bora Bayraktaroğlu: Breast reduction is a special aesthetic surgery procedure that reduces the physical and emotional weight imposed by overly large breasts and creates a lighter, more balanced, younger and freer silhouette. When performed on the right patient with the right plan, the result is felt not only in the mirror but in every moment of daily life.
